What Causes Gynecomastia in Men

Most cases trace back to a shift in the balance between testosterone and estrogen. That shift can happen at different points in life:

  • Puberty, when the tissue sometimes never fully settles
  • Weight gain, which adds fat to the chest
  • Aging, as testosterone levels drop
  • Certain prescription medications
  • Anabolic steroids and some supplements

Many men live with it for years because it does not respond to bench presses or push ups. Surgery is the only way to remove glandular tissue once it has formed.

Gland, Fat or a Mix of Both

During your consultation we examine the chest to see what is causing the fullness. Firm, rubbery tissue right behind the nipple is gland. Soft tissue spread across the chest and toward the armpit is fat. Fat alone is often called pseudogynecomastia.

Most men have a mix of both, and the plan depends on that mix. Fat responds well to liposuction. Gland has to be removed through a small incision. When the skin has stretched, a skin reduction may also be part of the plan.

You go home the same day in a compression vest that keeps swelling down and helps the skin settle onto the new chest shape. Short walks start right away. Most men return to desk work in about a week.

Lifting, chest workouts and running usually wait 4 to 6 weeks, and your surgeon clears each step at your follow up visits. Bruising fades over the first two weeks, and swelling keeps going down for a few months.

Results You Can Expect

You will notice a flatter chest as soon as the vest comes off, and the final shape shows once swelling resolves, usually around three months. Gland tissue that has been removed does not grow back. Large weight gain, steroid use or some medications can bring back fullness, so a stable weight protects your result.

Gynecomastia Surgery FAQs

Can gynecomastia go away on its own?

Gynecomastia that starts in puberty sometimes settles within a couple of years. Fullness that has lasted longer than that, or that has turned firm, usually stays until the tissue is removed.

Surgeons often grade it from mild fullness around the nipple, to moderate enlargement across the chest, to larger enlargement with loose skin. The grade guides whether you need liposuction alone, gland removal or a skin reduction.

Liposuction uses a few tiny openings. Gland removal uses a small incision along the lower edge of the areola, where the color change helps hide it. Larger cases with skin removal need longer incisions, which we go over with you before surgery.

The tissue we remove does not come back. Keeping your weight stable and avoiding steroids and supplements that raise estrogen protects your result for the long term.
